1. From ARTA Lending main menu, select the Setup section.
2. Select Credit Life.
3. Select the insurance plan you want to print.
4. Click Print.

Please fax your materials to (800) 860-3595 Attention: SupportLine.

Please include the information listed above (Institution Name, Name, Phone, BSI Customer Number, Software Version #, E-mail Address and a Description) along with your fax.
